Chronic Subdural Hematoma

Chronic subdural hematoma fluid may be xanthochromic (yellowish) or dark brown colour.

A membrane forms beneath the dura and above the arachnoid membrane, which is semipermeable and seepage of fluid & altered fluid, collects inside the membrane.

Chronic subdural hematoma occurs in extremes of aged.e., in infancy and in the elderly.

Trauma is the commonest cause, but very commonly, trauma is so trivial that history of trauma is not always obtained. The duration of formation of chronic subdural hematoma is 3 weeks. Between 1 to 3 weeks, hematoma is subdural.

The predisposing are cerebral atrophy, alcohol, coagulopathy, etc.

Due to cerebral atrophy the bridging veins from cerebral cortex to sagittal sinus are stretched. The stretched veins are torn whenever there is head trauma.

The common presentation are headache, memory deficits, hemiparesis, deterioration of conscious level, etc.

Non-contrast CT scan (NCCT Head) is investigation.

Surgery is the treatment of choice. Normally frontal and parietal burr hole evacuation of hematoma is done.
